Clinic Entry

Please include the following with your entry form:

  • This completed and signed entry form (riders under 18 years need a guardian to sign).

  • Legible copy of negative Coggins Test within 12 months & proof of Flu/Rhino.

  • Clinic Entry Fee (Allie Anderson’s Venmo: @Alexis-Anderson-103 or contact to arrange other form of payment)

  • Signed Release Forms (riders under 18 years need a guardian to sign).

Special Notes – 

  1. Preference will be given to complete entries- PLEASE DO NOT SUBMIT ENTRY FORM WITHOUT PAYMENT.

  2. Cancellation/Refunds – no refund unless spot can be filled.  

  3. Safety – wear approved helmets and appropriate clothing.

  4. Dogs not permitted.
